I. Health Examination Details
Examination Dates: March 13 (Thursday) & March 14 (Friday) – Please select one day to attend.
Examination Time: 07:30 AM – 11:30 AM • You must fast for at least 8 hours before the examination.
- Check-in Location: 6th Floor, Holistic Education Building
- Health Examination Location: 6th & 7th Floors, Holistic Education Building (Check-in is required before proceeding)
- X-ray Examination Location: 1st Floor, Holistic Education Building (No check-in is required; please proceed directly with your examination form and queue.)
- EKG & Thyroid Ultrasound Examination Location: B1 Floor, Building N (Objective Structured Clinical Examination Center).
(Take the stairs next to the Hi-Life convenience store to B1. No check-in is required; please proceed directly with your examination form and queue.)
II. Required Forms
Please verify the accuracy of your personal information. It is recommended to complete all forms in advance to reduce waiting time.
- Name Stickers (6 pieces)
- Fecal Sample Collection Kit
Please fill in Name, Age, and Collection Date on the label.
- Health Check Process Form (Printed on the reverse side of the Physical Examination Record for Laborers)
Signature required at the bottom of the form.
- Physical Examination Record for Laborers (Printed on the reverse side of the Health Check Process Form)
Please complete the form in advance.
- Personal Information Consent Form
Check the agreement box, sign, and fill in the date at the bottom of the form.
- QR Code Survey Form
Please complete the Musculoskeletal Symptom Questionnaire and Abnormal Workload Assessment, then sign and fill in the date at the bottom of the form.
- Thyroid Ultrasound Examination Form (Applicable only for those who have opted for the self-paid thyroid package.)
III. Examination Guidelines
Dietary Guidelines:
- Avoid staying up late, consuming alcohol, or eating highly stimulating foods for three days before the examination.
- Ensure you fast for at least 8 hours before the examination (small amounts of water are allowed) to prevent inaccurate blood test results.
- If you are taking chronic medications, continue taking them as usual (except for blood sugar-lowering medications, which should not be taken on an empty stomach).
Dress Code:
- Wear a plain T-shirt (avoid clothing with zippers, buttons, sequins, plastic, or metal accessories).
- Female participants should wear a sports bra without an underwire.
- For the EKG examination, avoid wearing one-piece dresses or tights.
- For thyroid ultrasound examination, avoid wearing high-necked clothing.
For Female Participants:
- Do not provide a urine sample during menstruation (three days before or after). Please inform the check-in staff.
- If you are pregnant or suspect you may be pregnant, please inform the check-in staff and avoid X-ray examinations.
Additional Reminders:

- Rest for at least 10 minutes and relax before your blood pressure measurement.
- During blood drawing, stay relaxed, and after the procedure, press firmly on the puncture site for 5 minutes without rubbing.
- If you wear eyeglasses, bring them along for vision tests.

- Submit samples within the examination hours.
- Store at a refrigerated temperature (0–4°C) for up to 2 days (Do not freeze).
- If you have opted for additional self-paid tests, please complete the payment by March 5.
- If you have experienced discomfort during past examinations (e.g., dizziness from blood draws), please inform the Office of Environmental Safety and Health in advance for assistance.
